Certain pragmatic trials lack controls to ensure that observed differences aren't caused by biases during the trial.

The authors of the Pragmatic Free Trial Meta identified RCTs published from 2022 to 2022 that self-described themselves as pragmatic. The PRECIS-2 tool was used to assess the degree of pragmatism. It includes areas like eligibility criteria, recruitment flexibility, adherence to intervention, and follow-up. They discovered that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains.

Trials with high pragmatism scores are likely to have broader criteria for eligibility than conventional RCTs. They also contain populations from various hospitals. The authors claim that these traits can make the pragmatic trials more relevant and applicable to daily practice, but they do not necessarily guarantee that a trial conducted in a pragmatic manner is free of bias. Moreover, the pragmatism of the trial is not a definite characteristic; a pragmatic trial that does not possess all the characteristics of an explanatory trial may yield valuable and reliable results.
